To purchase individual health insurance:
- Decide which insurance company and health and dental plan is best for you.
- Fill out an application form. To do this click on one of the "GO" buttons below,
print off the application and then fill it out (we can also
mail you an application form).
- Mail or fax the completed health insurance application to either us or the
insurance carrier (depends on the carrier), and:
- You may also have to include a cheque (payable to the insurance company)
for the first premium installment, or a void cheque, etc. This will be outlined in the application form.
- If approved you will be mailed your insurance policy*. Usually coverage commences on the first day of the month following approval of
your application.

if your monthly premium has been adjusted and is not what your were quoted. If
your monthly premium has been adjusted you have the option of refusing your policy
(and if you have mailed a cheque you will receive a full refund from the insurance company).
