The unique feature about this product is that no medical questions are asked if you are applying for coverage within 60 days from your employee benefits termination date!

Purchasing Health Benefits

• Print an online application form, fill it out and mail or fax it in.  You can also download a form and then print it at your leisure. 
• We will contact you when your request for health benefits has been processed. 

* No medical required if applying within 60 days from the termination of group insurance.
